Long post, so the short version is here: As long as the Achievement System in D&D gives nothing but a title that makes people smile, all is well. You have been warned about how long this post goes on, so read on if you dare.
Like most things, there is a potential down side to an Achievement System in D&D. People might get pretty stuck on getting them sometimes...
In D&D, so long as the Achievement System such as it is does not award anything other than a title that makes people smile, it should be fine. I doubt anyone will go crazy trying to get one.
It was all about coming up with fun ideas for me, no actual rewards. I did think while making them that it could ruin the game if people actively tried to achieve some of them, so I hope that's not what people do.

This World's Not Done With You Yet - Cast Revivify on a Teammate.

Make Me Famous! - Get the party referenced in a local paper/noticeboard or in a town crier's speech.
You Spill My Drink, Partner?- Start a Bar Fight.
1st Edition Troll - As a Rogue, steal a Spellcasting Focus from a party member.

Give it your best: expend all regenerable resources in a single encounter
Empty the arsenals: expend all resources in a single encounter
Life gambler: perform an offensive action that cost hit points (not sure if such actions exist, so it is up to the dm)
Self sacrifice: die in a fight, the rest of the party survives
tis but a scratch: refuse healing at 1 hit points
preacher: convince a large group of people to turn to a religion of your choice within 1 hour
godfather: convince someone that they should not fight in the middle of a fight
god's wrath: use divine intervention to kill a noncombatant
light of the atheist: let a follower of a god turn to atheism
I am my true king: disregard a quest
tired of godhood: reach level 17, willing retire as an adventurer, live a commoner's life
getting help: giving another adventurer a quest
pass on the spirit: train another adventurer
undying might: get turned into an undead creature after getting killed, then fight and win as an undead creature
exotic revival: use a party member's remains to create an undead creature
battle-scared: use a prothetic limb to replace a limb lost in battle
veteran's wounds: become more powerful as a direct result of being damaged
We're all mad here: gain an indefinite madness.
