spell attack is when the description of a spell specifies that you have to make an attack roll. It involves your mod and proficiency bonus
you do not combine them
for bonus, the spell save dc that you see to the right of the attack is the number an opponent has to beat to receive either no damage or half damage based on a combination of spell description and opponent features. If you do paper, the dc formula is 10 + spell mod.
On my character sheet for Spells, I have a +5 modifier and a +8 Spell Attack...
