Hades is a place that's so soul-crushing, even the Yugoloths can't stand it and moved out of it, and they're native to it. The Abyss is less evil by comparison. Evil mixed with Chaos is not more evil than Evil mixed with Law, and it's definitely not more evil than Absolute Evil.

to me its the abyss by far, just the idea of it madding horror screams of lovecraft horror. Just the sight into it can drive men mad. That is by far i think the worst of the worst
The abyss is really not a single plane. There are layers of the abyss that are more tolerable than others. But then there's also surely a layer of the abyss that beats the rest out in terms of suffering. I marked Carceri because it's almost impossible to escape, and contains some of the worst offenders from the other planes.

all outer planes are like this, some have more some have a set number. ie 9 hells, 7 heavens etc. The abyss has over 670 layers, and possibly a unlimited number. (not that anyone could survive to count them all.)
And yes Tartarus (old name for Carceri) is difficult to leave, and is a prison for some real prime evils, it is also a fairly mellow place as well. Each of it's layers (string of nested planets that keep getting smaller and smaller) is more stable and understandable than most areas or the Abyss. The chaos in the Abyss, and the high level evil gods are what make it terrifying.
Also the single scariest D&D entity is the Pale Night ...
Each plane is tailored to be the worst possible for the souls sent there. It's all hell, designed to inflict the optimal soul-crushing punishment on the poor sods who didn't do well in life.
And conversely, the upper planes are all perfectly tailored to provide the best possible afterlife for the good guys.

The small minded will believe the Joker is worst. But, honestly, the Joker’s evil rarely reaches beyond Gotham. His influence rarely reaches beyond wherever he is.
To get true evil, you need organization and influence. You need power, not the power of a fist or a gun, but of corrupt institutions.
Hitler would have been a second-rate painter if he never got followers. Pure chaos doesn’t permit the kind of power Hitler or Lex Luthor had/has.
The Abyss will never reach the depths of evil Hades can.
