Just to clarify a couple things, at least from the perspective I have been trying to explain:
When someone uses such a term, perhaps they really are doing so for flavor or perhaps they are doing so with expectations of it meaning something, or it having some noticeable effect (no pun intended). If the former, no worries, carry on. If the latter, though, it is important for the DM to have some clue what those expectations are. That does not need to mean any precise detailed explanation from them, but at least an idea that can be worked with and discussed further if need be.
[Redacted] My position is that there should be more dialogue over such things, anything unclear about a character that looks like there might be deeper expectations attached to it, really.
