Attacking someone behind cover grants them a bolt to AC rather than subtracting from your To Hit Bonus. Unless you're just after the least likely to hit scenario?

In Valda's Spire of Secrets there's a spell called Transient Bulwark that gives the next attack against the spell's target a -10 to hit. There's also 2014 GWM/Sharpshooter.

Strength Drain can result in a stat dropping to 1.
Using a non-proficient weapon, they can attack with a -5 to hit, and disadvantage could be applied various ways.
There's also Bane and Cutting Words to further reduce the attack roll
Ooh, right! Also attack a creature behind 3/4 cover. Maybe throw in Mirror Image?
MI technically doesn't interact with the attack roll itself, so might be a little outside the scope of this.
