My character (all the characters in my group) just hit 16th level. It's a homebrew campaign that's been running or a couple of years. We started off counting XP each session, and then some time after level 10 switched over to landmark or "story arc" leveling.
However... there are six players in the group. We all started the campaign fresh at level 1, but some characters died during the campaign, at which times the player would create a new character at the current party level and the DM would introduce the new character when appropriate in the narrative. So, we have all run a character from level 1 to level 16, but for some of the players, it was more like "character A from level 1 to 9 and character B from level 9 to 16".
So I'm not sure if that would technically meet the requirements of your question.
Gotta say though, I really thought a Tier 4 character would feel more powerful than it does. Sure, we're practically gods compared to level 1 characters, but we're going up against Tier 4 enemies now. Sure it feels good to have Holy Aura, Heal, and Fire Storm in my back pocket... until the DM says we're facing off against a Neothelid, a couple bodaks, and a few random demons thrown in just for good measure.
And we're hunting an illithilich. Or he's hunting us. One or the other.

How many players here have had a character reach tier 4 after starting from level 1? And what classes and subclasses were they?
Not yet, my group has multiple campaigns that we bounce between. They are all in mid to late tier 1.
