My group is nearing the end of the Rise of Tiamat adventure. This will get them to 15th...maybe? What is out there that takes them from 15th to 20th? I know Mad Mage will hit 20, but it starts at 5 and I'm looking for some sort of smooth continuation for my players. I've looked at various sites and there doesn't seem to be much to fill that gap, that's easily found at least, between 15 and 20.
Tomb of Horrors? Never played it but supposed to be ridiculously hard.
Tomb of horrors doesn't have a set level. Level 15-16 might be the lowest that could start it. It is likely to be lethal.
Dungeon of the mad mage is for level 5-20, but since it is a dungeon you can skip a few floors to get on their level quickly, or worst case, they breeze through the first 10 floors (might be a little boring or it might be a nice power trip after defeating a god).

I agree, it might be a nice change of pace after facing such high powered opposition. Blowing through the first few levels will be quick and feel like a vacation.

IMO, at level 15, having defeated Tiamat, it's time for your characters to get a little proactive about what they want for their endgame. They're Big Deals now. Wizards need to find apprentices, fighters need to build castles, etc. You should ask them to think about what they want. Then personally, I think you take that information and start building your own story with that end in mind, whether they want to conquer a kingdom or explore the planes or just find a bigger bad guy to beat up.
If you don't have time to write anything of your own, you could probably scale up a published one. I don't think there's an upper ceiling on how tough Avernus can be. And I think Adventurer's League publishes some endgame stuff.

If you're really mean it's time to introduce them to the real evil of the universe:
PAPERWORK.
Oh, you're a level 15 wizard? Time to get some apprentices right? Any idea how to manage a ledger for that wizard college? How about spending a night sifting through 300+ applications? Interviews? Then you'll have to negotiate rent on a pocket of the alternate plane to house the school. Or do you plan to just create your own? Do you really think the universe is okay with you just MAKING a pocket dimension without filling out a PT-2093 Schedule C form? I don't think so!
If nothing suits your fancy, and you're feeling creative/brave, you could always homebrew something?
As an option, you could reinforce the fact that whilst Tiamat is defeated, she is not truly destroyed. Tiamat may have been defeated, but she is not gone - and perhaps neither is her cult, the survivors of which will swear bloody revenge on the heroes.
With the potential of Tiamat returning sometime in the future, the only way to truly stop her from ever threatening the Material World is too destroy her in her own plane - within the nine hells...
But can Gods be defeated by mere mortal weapons? Or are there rumoured artefacts that the council of waterdeep members know about? Artefacts that have the power that could challenge a Deity? (time for a fetch quest!)
Just some ideas to consider - happy gaming!

At lvl 15 Tomb of Horrors is a joke.

Here's the DM's Guild's list of lvl 16-20 AL adventures (WotC hasn't printed anything in hardcopy though).
IF they've defeated Tiamat, it's time for those characters to retire as saviors of the multiverse and roll up new characters.
