I was having the same problem. Tried the same things you listed. Still didn't show up, but then after clearing my history/cash, the map pack is now in my VTT (quickplay and adventure groupings).

I wanted to mention that you CAN drag Pral into the encounter (with the right token!) and then get rid of the incorrect Bandit captain. I don't see any way to get Bandits to have the "correct" token as new players will see on the physical map.
I will also confirm that the Heroes of the Borderlands D4 does not have correct roll results.
Okay, I have synched my entitlements and see the source book for Heroes of the Borderlands and the dice set listed in the products I own. What is missing from that list is the Heroes of the Borderlands Digital Adventure Pack. I double checked and I did purchase that.
As a result, when I go to the Maps VTT, I do not see the maps for this adventure and I'm also not finding the premade characters.
The issue here appears to be that the digital adventure pack is not yet showing up in my entitlements. I did receive an email message yesterday saying that my digital adventure pack had arrived, so something is definitely amiss here.
Is someone able to check my entitlements and get the adventure pack added?

I found out later that if you select quick play on a map it comes with all the tokens but that you have to create a campaign or use it in one you already have. I assume this is working as intended and there isn’t meant to be a premade campaign but if that’s the case then the quoted text above is poorly worded.
So in this article https://www.dndbeyond.com/posts/2059-how-to-get-started-with-the-heroes-of-the on step 3 it says we can choose the premade character made for the D&D 50th anniversary, which is a bit disappointing, thought they would create an exclusive premade character for the starter set, or limit the creation of characters to the 4 classes (with 2 subclasses) from the starter set...

Just saw a callout saying the premade characters will be available in a few weeks... Still disappointin,g this is not available for the release...

The DDB version of the book is the digital adventure pack. I assume they refer to it that way simply because it doesn't have everything included in the physical box (tokens, cards, etc).
If you have access to the book, though, you should have access in Maps. If you've already checked that your Account Entitlements are in sync and that the book is listed under Current Product Licenses (https://www.dndbeyond.com/account/licenses) and still don't see it there, you may want to contact customer support (http://dndbeyond.link/SubmitTicket).

This! For $60, we should have physical printed maps.
If you spent that much, then you must have purchased the Ultimate Bundle, which includes both the physical and digital versions. That means you are indeed getting physical printed maps.
Digging a bit deeper, the Pral monster stat block should be : https://www.dndbeyond.com/monsters/5783457-pral
It's somewhat important, both for the imagery, and because Pral doesn't have a Pistol and the Bandit Captain does.
Another issue: Vinx appears as a blank silouette token, despite linking to the correct stat block:

Seen on Cave D: Goblin Lair
Narthus and Ivlis on tthe Cave K: Shrine of Evil Chaos also show as blank silouettes with the correct stat blocks.
Someting is very wrong with Ivlis especially:
Statblock says HP 49, but the token says HP 55.
And when I add Ivlis manually:
The token appears and the numbers match.
UK customer so used product key to activate - cannot see the maps for this module in maps app

I had the same. Use the "Sync My Entitlements" button as mentioned above and it should work fine.
