Does a hag’s CR include its lair as a factor, or do I need to calculate a new CR based on the fact that she has a lair? I have a hag who is well-established in her lair (I’m using an annis hag), and it seems like lair actions would significantly increase the difficulty of a fight. She doesn’t have a coven yet.

An Annis Hag typically does 45 average damage per round with its multiattack. When calculating a monsters CR with lair actions you just assume the lair action hits one person if it's a single target lair action, or 2 targets if it's capable of hitting multiple targets.
The Annis Hag's lair action would deal 10 (3d6) damage on average, we assume this hits 2 people making it deal an extra 20 damage per round for the purpose of the calculation.
This extra damage increases a hags offensive CR from 8 to 10. The defensive CR doesn't change. Therefore the overall CR of the Annis Hag increases by 1, giving it a challenge rating of 7.
If you want to be pedantic you could say a smokescreen would increase it's defensive CR by making it harder to attack, I'd rule that it'd only increase the defensive CR by 1, which would give it an overall challenge rating of 7.5, rounded down to 7.
