So, the wizard Bowgentle is kind of an important character in my game, but one player could never remember his name and kept calling him "Bojangles". Eventually I introduced a pet dog named Bojangles which got a surprisingly big laugh. In the same town there was also a merchant who had a pet that was a large, fat, lazy orange tabby named "James A.", and no one got the joke. The same thing happened with a previous campaign where I was a player and my character had a pet goat named "Thaco" and no one knew what I was referencing.
I named the accident-prone Evocation professor at a school of magic Pyrrha Ignatius, but only one of my players (she studied languages in college, and then taught them to me; she’s my mom!) got the joke. Maybe it’s too obscure, but I really thought that it was obvious that both names mean fiery!

I live with several severe autoimmune conditions. If I don’t get back to you right away, it’s probably because I’m not feeling well.
DM: "You walk into the tavern, and see a cloaked figure sitting at an otherwise empty table. The figure briefly glances up at you, and with a crooked finger, beckons you closer."
Players: "Who else is in the tavern?"
DM: "Uh...there is a goblin...named...Boblin."
Players: "I want to talk to Boblin the Goblin."
Side note: One of my fellow party members is a Tortle Monk. Any ideas what his nickname is?
I as of late found this issue. Utilized a name generator on the web, discovered the name Negrid. Thought it was a beautiful cool name, until I utilized it and my players were shocked. "WHAT is her name?! 192.168.100.1192.168.1.1
Wizard npc has a simulacrum called Simon. This simulacrum has been around a long time so looks much younger then the wizard so they haven’t twigged yet what it is (might not until they find out the friendly, absent minded, not very powerful wizard is actually the bbeg)

The players skipped encounters with them thus far, but I plan to have a pair of incompetent bandits named Lohzar and Felyar.
I also have an eccentric wizard NPC named Mori Faldna. It's an anagram of Florida Man. His tower is protected with a moat full of alligators.
