Ultroloths are yugoloths, meaning they are typically neutral evil. The Monster manual says that they seek to "hoard power and spread suffering."
Do you want the war to be a "good vs. evil" kind of thing or a "war is hell and there are no heroes" kind of thing?
For the first option, it could be the ultroloth corrupted a hero long ago into becoming a fighter lich, manipulating them to use their Undead forces to the ultroloth's benefit, with the other side of the war being whatever fantasy kingdom you want. Maybe the ultroloth was slighted by the rulers of the kingdom in the past, or hopes to take over the kingdom by disposing of the fighter lich after they conquer it.
For the second option, it could be the ultroloth acting as a double agent for both the demonsand devils to aggravate the blood war, with lots of mortal casualties on the side. Perhaps the ultroloth wants some relic that a demon lord or devil is in possession of, and hopes that by making the war more intense, that fiend will be forced to use it, allowing the ultroloth to swoop in and steal it. In fact, the monster manual lists as a potential "Ultroloth Conspiracy": "Provoke hostilities between immortal armies and sell magic weapons to both sides."
would recommend establishing that their are two factions (preferably large, kingdoms, empires, etc), that already are at each other's throats, possibly even already at war. The ultraloth comes in and secretly deals magical, hellish weapons and fiendish mercenaries to both sides, simultaneously increasing the suffering caused by the war and making it difficult for one side to decisively win.

I have an Ultraloth trying to start never-ending war, and I cant really think of how or who the war is between.
I kinda want to use pointy hat's fighter lich if anyone knows that, but its not a must
