My party just discovered this heart and I was curious of how the heart would work. Does it kill the person and the trifling take over the body? Or does it kill the host as well? I am trying to figure out how things will play out.
I recently played this :). We found the withered heart. Used detect magic to figure out it was magical. The monk really wanted to attune to it. The rest of the party said "are you sure you don't want the cleric to identify it first?" Might be a good idea! Adamant monk in a risky mood refuses ... we take a short rest. Monk instantly dies. Heart pops out and shrivels in his hand. Sudden open heart surgery by the rest of the party restores the heart to its correct place and the DM allows the revivify from the other cleric to bring the monk back. Monk hopefully learned a lesson :)

It's a trap item. It kills people and that's it.
That’s what I figured. Already have a warlock running around with a long sword glued to his hand, what else can they get themselves into.

Identify does not tell you about a curse normally. Only for a few select items that say so.
