You can write down the name of any specific and sentient being that you dislike on the book’s pages. The being's detailed face appears drawn on the page. The picture is based on your memories of them. It only works if you actually dislike them. You must have spoken with them at least once and you must have seen them with your own eyes.
Each day, the book sends each pictured being a loud telepathic message in your voice at early dawn, letting them know that you dislike them. In addition, the book allows you to cast the Sending spell once per Long Rest. You can only target creatures whose faces appear in the book.
A Remove Curse spell cast on a victim breaks the link between the target and the book, removing their face from the book’s pages forever. After the curse has been lifted, the same person cannot be targeted again, unless they do one of the following things to you: Steal money from you in any way, spread nasty rumours about your spouse or make untrue claims to other people that they bested you in a game
I personally like magic items that are potentially strong, but without a clear purpose. Given the right player and the right circumstances, this could be really useful. And also, such an item can create amazing RP moments in the right hands, which is useful on its own. IMO they are also believable lore devices. There was actually an enchanter or a buyer who was so bitter towards someone, that they wanted something like this to exist.
This is an item that would probably be replaced at some point if you can just select magic items at a store. But in many adventures you have to make use of what you've got and think out of the box.
Take for example someone like Strahd von Zarovich. Imagine being able to torment Strahd with a daily insult and also send him a telepathic message once per long rest.
Oh god. I've had a player make liberal use of message to harass NPCs. The idea of automating that process is so annoying, in all of the right ways. I would have the announcement time settable, so that it can occur in the middle of the night. Though it wouldn't be enough to disrupt a Long Rest mechanically, it could be used to mess with the victim's sleep enough for roleplay purposes.
Now, just get a 20th level Artificer to attune 6 of these and spam the same people every 4 hours.
Can you imagine if this were a real product? Political figures would be bombarded non-stop. It would genuinely be enough to drive someone insane. Of course, they would have have Rings of Mind Shielding for this exact reason.

I have to say, I'm really proud of myself for how oddly specific the re-cursing conditions are. 😂 I can just imagine what led to the creation of this item.
I've got a West Marches-style session coming up, so I think I'll change my plan to include a powerful NPC who creates magic items with exactly those kinds of weirdly specific rules. No one will be quite sure whether they are filled with ancient wisdom, or just a quack.
