A while back I saw this awesome artwork and instantly wanted to build it as a Dungeons and Dragons weapon. I spent a few days crafting it and this is the final product, please tell me what you think! Link to my naturalcrit where I posted it here
Weapon (any), legendary (requires attunement)
This blade is a thin clear bottle, with a cork attached by string and winged crossguards made of glass. Inside the bottle is a swirling, black and purple mass of mysterious substance not bound to any surface, in the center of the mass is a single, unblinking, pure blue eye.
Take Form. As a Bonus Action, you can uncork the bottle and speak the name of any weapon. The ichor within the bottle will pour forth and take on the form of the named weapon. Once the swirling mass has taken form, it must take a minute to recharge inside the bottle before it can take a new form. Though it can hold any form for an hour.
Dark Origins. While you are attuned to this item, damage and attack rolls with it have a +3 bonus. Additionally, all damage dealt by this weapon is psychic rather than the damage of the regular form.
Mental Strike. When you reduce an enemy to 0 Hit Points with this weapon, you can force them to make a DC 12 Constitution saving throw. On a failure, it has its Intelligence score reduced to 0 and falls into a catatonic state that it can only recover from through a Greater Restoration spell or a long rest. Once this feature has been used, it can not be used again until after the next dawn.
Brilliant Sacrifice. You can use an action to break the bottle in your hand or on a solid surface, performing a brilliant sacrifice. The bottle is destroyed and releases its encased ichor in an explosion that expands to fill a 15 foot radius sphere centered on it. The creature inside, now free, possesses the body of the sacrificer or closest humanoid. Whoever is possessed must make a DC 20 Constitution saving throw. On a failure, they are possessed and their soul, devoured. On a success, they are reduced to 0 Hit Points and knocked unconscious. Every other creature in this radius must make a DC 15 Intelligence saving throw or take 12d6 psychic damage. The possession feature of this action does not affect creatures with Legendary Resistances.
The Essence of Nightmares. If you fail 3 death saving throws, or die from damage while the Blade of the Great Old One is in your possession. The swirling mass will pour out of its container and into your body, allowing you to fight for an additional minute, after this time has passed your soul is devoured and the essence retreats back into the bottle.
For the essence of nightmare part what if someone were to heal you or use resurrection magic before the time is up? Does this weapon get a telepathic bond with the user? And does it have stats (and HP or a way to destroy it) and perception checks like other living weapons?

Ooh interesting, that thought hadn't crossed my mind. I would imagine at that point it would probably force the ichor out of that it was possessing and back into the bottle, I don't imagine it'd be too happy about it though.
I purposely avoided giving this one sentience because I create too many sentient weapons haha, plus I think it seems creepier as a silent surveyor. As for destruction I'd say the basics of getting rid of any other magic item you can't destroy, perhaps banishment, and it's not exactly living? per say, more along the lines of basic sentience and desires
Really creative, good utilization of just some fun art. Is there any sort of alignment or god involved, at least in your idea of the weapon?
I plan on having it be a loot drop after a series of puzzles in the end chapter of a campaign surrounding Dendar the Night Serpent, so it will most likely tie into the primordials in some sense. Though I don't see it becoming anything more than a prized weapon, as the essence inside is not smart enough to act on its own accord.
