The target must make a Wisdom saving throw. On a failed save, the target takes 8d10 Psychic damage and has the Blinded and Deafened conditions for the duration. When this damage is dealt and in any of your turns while a target is still affected by this spell, you can choose to take 2d10 Psychic damage. If you do, that target also has the Paralyzed condition until the start of your next turn. On a successful save, the target has the Frightened condition until the end of its next turn, with you as the source of the fear.
At the end of each of its turns, the target repeats the save, ending the spell on itself on a success.
Using a Higher-Level Spell Slot. You can target one additional creature for each spell slot level above 6.
It's Hold Monster with a side of extra damage and fear on a success, so 6th level seems fine. Just so you know, the Frightened condition already prevents you from approaching the source of your fear, so that is completely unnecessary to say.

That is a big problem with 2024's "the target has the Frightened/Charmed condition". It doesn't actually say who's the charmer or what the source of the fear is. Though it does seem to be the assumption that whichever creature or effect created the condition is the source.
