I made an axe that instantly cuts down a tree up to 5feet thickness. It's named Rizzrack's Axe . (aka. Timbersaw the Dota Hero)
The flavor text goes:
Forged by a Goblin that really, really, really, really, really really, really really, really, really, [...],really hated Trees.
Unsatisfied to only kill one Tree at a time he set out for a different way, a better way to get them. BEFORE THEY GOT HIM!
Another "magic" Item that worked out rather well was a mis-shapped triangle "related to some ancient god" I really didn't think much of it putting it in the store, however the Halfling Rouge in the party was fanatic about getting it. Stole it from the store and has been checking it every day. So I did what ever good DM would do and made it an object of an old God. He started to hear whispers and see things in the dark. It unlocked the door to a chamber of a Faceless one and if he doesn't forget it there I am gonna make it slowly summon things around him and eventually plunge them all into the Underdark.
I like the idea of a Ring of Rememberance. When you put it on, you're made to look like the last person that put it on until you take it off. But if you put it back on, you'll be the last person that wore it so it wouldn't do anything! Could also be useful in some situations.

Had a player from another group tell me that she used a wish spell to create a "Kidney Gun" . The only way to collect ammo is via grappling hook, i think half of her party has only one kidney or something at this point.
I personally like items that do appearance altering stuff. So like a Heavy Flail that increases the weight of the player attuned to it by 50lbs. I called it the "Weight of the Task Master"
Another one I have is called "Sword of D'Artagon" , it is a rapier that once you attune to it you grow "Musketeer" facial hair and hair styling.
A Leather Jacket that can be worn over armor, while wearing it any personality flaws are magnified.
My personal favorite is a Harmonica I called "Soul Flame", playing it will basically use the Firebolt cantrip up to 4 charges. But only if you play specific song such as Karma Chameleon on it. To gain charges back you have to play a specific song at dawn. I choose the opening riff to When The Levee Breaks.
I gave one of my players a sentient tome known as "The Book of Glory". It has information on man things, but it has to roll to find them, because it's not that intelligent (Imagine Wheatley from Portal 2).
I also gave another player (warlock) a book of the elder gods. It increased their charisma and allowed them to create weak, temporary thralls. However, it decreased their wisdom, and sprouted tentacles from their back.
I created a Bunk Bed Figurine as loot. It expanded into a full size bunkbed when thrown and the magic words were said. Then shrinks back down when the magic words are repeated.
They once used it to pin a pegasus to the ground (it kept failing strength checks to free itself). They eventually lost it to a gelatinous cube.
I also enjoy the idea of magic items having very definite positive and negative effects, like the whip on the first post. Something similar could be an anti gravity mace, that does extra force damage, but then the target has the effects of the "fly" spell on them for a number of seconds equal to the force damage dealt. I'm not sure of numbers b/c of balancing but that's my idea anyway

Basically, it would be magical napalm. It would stick to everything in the blast radius, and make the terrain slick while the fire burns everything. Pray your party is not caught in the blast.
I left a few of my personal custom items over on the homebrew inspiration thread one of which is a magic bookmarks that has a date light up in red when your taxes are due LOL. I find seeming useless items fun overall.
Kobold Press has some interesting items and trinkets in the articles section that can be changed to fit a lot of things. one of which is a ring that lets you rage but if you fail check you just start attacking everyone at the end of a battle. it can also force you to eat the guys you just killed and start doing blood letting sacrifices.
A fellow DM once asked for my help to stock a "sketchy magic item shop". The shopkeeper was an evil fey and liked to make mischief. Here's what I came up with:
The Ever-Disappointing Mug. This utilitarian steel traveling mug is cursed. If a creature tries to drink from it, the drinker is subject to its curse. While cursed, the drinker will always drink from the mug, even if intending to drink from another source. The mug will appear in the drinker's hand and will be filled with their 5th-favorite drink. Manually filling the mug with another liquid will appear to work, but when a draught is taken from it, the liquid will transmute into the drinker's 5th-favorite drink. If the drinker attempts to drink from another source without using their hands, they will find the mug placed at their lips, preventing them from drinking from another source. Throwing the mug away will not prevent it from appearing the next time the drinker attempts to imbibe fluids.
The Uncandle. This six-inch tall, one-inch wide candle is made from black wax. Its wick cannot be lit. When the shadow of an unlit match or twindertwig passes over the wick, the uncandle creates an area of utter darkness centered on itself with a radius of 20 ft. The wax of the uncandle slowly disappears, as if it was melting, while it creates this area of darkness. Every hour it creates darkness, one inch of wax disappears. Blowing on the wick or briefly pinching will make the darkness disappear and stop the uncandle's slow disappearance.
