So what's the plan? Follow the Ghouls? Turn back and check out what the crab sign meant? Or investigate this room, all there is really is an ear shaped crevice in the wall.
There's nothing intrinsically out of place with the ear, in fact it seems to be a simple outcropping of rock. But you get a strange religious like feeling feeling from it, your Warforged senses seemingly more attuned to the divine in some ways than other humanoids that have lived with the gods watching over them since birth. This ear shaped area seems to be calling you to listen.
As you talk into the ear shaped crevice, your words come back at you meshed together in chorus, like a song. As this harmony of voices echoes throughout the cavern, you feel yourselves guided by one or many of the Sovereign Host. Mome would recognise the influence particularly of Boldrei, the god of community and home. You see an image in your minds from the distant past: of minors singing into this crevice, and being guided by Boldrei to make this place their home. But the minors fade to darkness and three dark figures are seen walking in and out of this place, with the Ghouls as their minions.
You all receive Inspiration from the Sovereign Host for listening to the spirits of this long abandoned mine.
"Ah, I see." Zoveldra remarks, finding this connection to the past fascinating. She rolls her shoulders before nodding to LANDMOP. "Yes, let's remove them for good. Their masters too if they're still here."

At this point I might introduce you to a rule I use for 5e. I offer 1 Action Short Rest a day. This allows you to get the benefits of a Short Rest in 5 minutes rather than waiting a whole hour. Is this something you think you might want to use now before heading into the next room with the Ghouls or save for later? And is Zoveldra scouting and sneaking still or are you all travelling as a group?
You all head down the mine shaft to the north, in the same direction as the ghouls. When you exit the shaft you come out into a grand cavern, even bigger than the ones you've seen so far. The cavern walls have clearly been extensively worked to extract all the Garlstone available previously. As you walk out into the cavern, you see the two Ghouls standing a few feet in front of two people. The first is a woman in her mid 30s with long, curly black hair and green eyes. Her fingers are unusually long and slender, and she moves swiftly and gracefully with fine blue boots and much jewelry. The portly man standing next to her is almost her complete opposite, with cropped gray hair balding at the crown; generally a pudgy and unhealthy-looking sort with a large wart on his nose. He wears a robe decorated with dozens of eyes.
The woman speaks to you in a stately tone, "I will give you one chance to tell me who you are and why you are here. Or my pets will have their way with you." She crosses her arms as the man beside her snorts. Zoveldra and Mome also notice in the corner of the cavern there is a red-haired man in splint mail with a mace and a fine dagger at his belt.
"Greetings" LANDMOP says nonchalantly. "My companions and I seek a lost dogooder. I'm sure they would also see you destroyed, but I believe compromise is in order. Tell us what you know of the cavern, its inhabitants and the gang that frequents here and we will leave you to your machinations"
