Between Trimyrai and Froskan, your party has information about the red slaad. (You can look over the stats by putting the mouse cursor over the name.) It is a being from Limbo (chaotic neutral outer plane), a creature of chaos and fury and infection. If you get wounded by its claws, you may need to visit a cleric for disease curing.
(I didn't expect that. The group rolled pretty good.) The slaad tries to bite Jack, then take 2 claw swipes at him.
Bite attack: 25 Damage: 7
Claw1 attack: 13 Damage: 5
Claw2 attack: 19 Damage: 9
Jack got hit once for 6 damage. He needs to make a CON save (DC 10) to maintain concentration on his spell, and then a CON save (DC 14) for the claw effect. (Jack has 2 DM inspirations at the moment, in case he doesn't like the save results.)
The party is now up. You may post in any order, but the actions will resolve in this order (with Froskan and Jack tying). If Jack makes his concentration save, the party will have advantage on all attacks.
The slaad is on the other side of the entrance wall. Only Jack can see it, at the moment. There is room past it (if you want to get 10 or 15 ft. away from it), but you have to get within melee range first, to get around it. (That means the first person to try to run around it will get the attack of opportunity by the slaad.)

Froskan mutters under his breath about stupid hellish creatures that are immune to cold spells as he rolls a ball of poison (level 2 Chromatic Orb) between his hands that he throws at the creature. He backs as far away from the creature as he can after launching the orb.

With the claw attack earlier, Jack sees that the slaad's claws are like sharpened turkey basters. As the claws pull back from the hit on his shoulder, he sees in disgust that a tadpole (half the size of his pinkie finger) is halfway into his wound. He hurriedly pulls it out, throws it at his feet, and squashes it. Then stomps on it several more times for good measure. (Used DM inspiration to pass the CON save.)
Player results for first round
Iskander - Casts the Hex and strikes for 13 damage. Would have missed, but the bright blue glow helped Isk pick a vulnerable spot to hit.
Trimyrai - Another precision hit for 7 piercing.
Froskan - The spell ball of poison has almost acid-like effects, as a section of the slaad's hide blackens and melts off (13 damage).
Jack - Music may soothe the savage beast, but Jack intends to inflict the pain (21 psychic). The slaad thrashes wildly and instinctively moves away.
Reverie - The slaad's wild movements throw off her attack. (A 13 just misses.)
Salad Slaad's second round
It used its Reaction to run to the back of the cavern. (There are no other exits.) That will give opportunity attacks for Jack, Iskander, and Reverie, which they can include with their 2nd round actions.
As it moves back to the party, you see it's flesh bubble with growth, and wounds rapidly close. (It regenerates 10 hp.)
It will claw Iskander, and then claw at Reverie, and bite at Jack. (The slaad moves away from Isk, but is still within 10', so Isk does not get another opportunity attack.)

The party did 54 points of damage the first round. It is visibly wounded, but the slaad regenerated 10 hp during its turn. The slaad is in melee range of Jack and Reverie, and 10' away from the others.
When posting your 2nd round actions, Jack, Iskander, and Reverie should include opportunity attacks from the first round. (Dissonant Whispers caused the slaad to run away, before it ran back on its turn.)
Iskander was hit for 8 damage. The Hex spell is still going (rolled 18 for concentration CON save), but he needs to make another CON save for the slaad claw effect.
All party members can post your second round actions. You still have Advantage on attack to-hit rolls.

(Last round, Jack's spell worked, so the slaad ran away, giving Isk the attack of opportunity. The chamber is small enough that it could only use 20' of movement to get away from Jack. For the second round, the slaad ran back towards the group, took a swipe at Isk as it was passing him, then moved to Jack and Reverie to do its remaining attacks. It is now back where it started, 10' away from Isk.)
