The general rule for polymorph is that it is limited to beast only. But the wording here says "or a creature it has seen within the last 24 hours (as chosen by the DM)" inplying a second option in addition to the normal polymorph rules. So if the DM allows it, the target could feasibly turn into the centaur

It says it was first created that way, not that it can only be created that way. But really this is 100% up to the DM. They can elaborate or limit the rules for its creation as they please (or even change them entirely)

I agree with Icon. Bear in mind that DM chooses the form, not the player (it is a poison not meant to be advantageous to the poisoned).
