A player in my group (Halfling Monk) obtained a Ring of the Ram. How would she use this item as an attack? Roll a 1d20 with a +7 to hit?

No proficiency bonus? Just the straight 1d20 +7?
Yup. I'd assume that the +7 includes a proficiency already, since the strength check is only +5. But either way, it's what the item says to do.

No, magical ring don't have that ! ☺ The ring makes the attack roll or Strenght check, not the character. That's why it's a baked in bonus.
Any disadvantage for being in melee range? It reads like it is a ranged spell attack.
It just says to make an attack. It doesn't specify ranged or melee so the ranged disadvantage in melee rule won't be applicable.
