Some spells and class features give you a different way to calculate your AC. If you have multiple features that give you different ways to calculate your AC, you choose which one to use.

AC calculations are independent. Bonuses can stack, but neither Loxodon nor Monk gives you an AC bonus. In a circumstance where you can access multiple calculations, you typically use whichever one of them you want, although it's possible one might have a wording forcing you to use the greatest. All bonuses stack with each other provided they come from a source with a different name, and usually all bonuses stack with any given calculation, but specific wording might mean otherwise.
Three-Quarters Cover: +5 (Cover bonuses come from the same source, Cover, so 1/2 and 3/4 cover won't stack)
Warforged racial AC bonus: +1
Carapace Simic Hybrid racial AC bonus: +1
Beasthide Shifter racial AC bonus while shifted: +1
Get the difference? Loxodons and Monks don't provide a bonus, so it wouldn't mean anything for them to stack. The easiest way to see this is to ask yourself, "if they did stack, what would my AC be, and why?" For a Loxodon Monk, there's no way to answer this question.

RAW, there is no way to combine the two AC calculations. A generous DM might homebrew the two combining, but there's no existing mechanic that allows it.

12 + con + wis + dex + a shield? Yes, that's probably OP, considering that with optional rules you could start the game with 17, 16, and 15 in those stats and increase at least 2 to 20 by the end of the game. Probably not absolutely the highest achievable AC, but one of the most easily cheesed.

No.
Neither of them simply add to your AC. Instead, both replace the standard calculation.
Loxodon: When you aren’t wearing armor, your AC is 12 + your Constitution modifier
Monk: while you are wearing no armor and not wielding a shield, your AC equals 10 + your Dexterity modifier + your Wisdom modifier.
They can't both be true, so you have to pick one or the other.
