Particularly for concentration spells, if it isn't immediately apparent. For instance, you cast Invisibility on a teammate to scout a hideout, but they wind up forced to attack and undo the spell without you knowing. Or perhaps you cast Slow on an opponent, who then teleports out of sight and makes the saving throw to undo the Slow effect a couple rounds later. In either case, should you be immediately aware that there's no longer a spell to concentrate on? I don't think there's anything in the rulebooks about this...
In the absence of rules, since concentration implies you are connecting your mind to the spell, I would rule that you notice when there's nothing to Concentrate on any more. You feel the shift in the weave.
Following what the other two have said basically. For a spell with no concentration or visible effect no one but the one making the saving throw knows. A spell with concentration the caster would know cause that concentration spot is now free they are no longer holding the energy or need to hold the energy to keep the spell going.

Concentration is part of the spell duration, if the spell has ended so has the concentration.
