So, I created a tiefling with a sentient tail. Mostly, I wanted some really good roleplaying opportunities, like the tail is wiser than the tiefling (Ala Jiminy Cricket), and the tiefling is jealous that the tail is smarter & more likeable.
First of all, as a DM, that sounds really fun to mess around with :)
Secondly, yes! I assume the tail cannot talk, but even if it can, I have a similar thing in a campaign I DM with my player's Great Old One Warlock. His patron is Cthulhu, and communicates with him using urges. For example, he will spontaneously get an urge to kill something, destroy something, spare someone, things like that. Occasionally, his patron would urge him to do something completely nonsensical, like to knock over a bookshelf or trip a random person. These things would generally be helpful later on, like with a secret passageway behind the bookshelf, or the person having information. Once, Cthulhu even urged him to spare the BBEG, since he was wreaking chaos, and that is Cthulhu's true end goal. It was a big moral question about whether or not to actually spare him, since the Warlock didn't even know why he suddenly didn't want to kill the BBEG, while the other characters still did. Some great roleplaying opportunities come from having a sentient item suggest a course of action, or even better, just DO something.
Maybe the tail could slap a guard or something sometime, getting them arrested and brought to a lord that ends up being in a bind the characters can resolve, or something like that. Just ideas here!! :)

If the tiefling and tail have an argument the tieflings tail could slowly convince the tiefling to collect a bunch of different items to make some sort of ritual (tail would make up an excuse like it would finally join their two minds) where what actually happens is the tail grabs a sword and cuts itself off and becomes a tiefling (with the ritual, idk what you could maybe transfer the tail to an automaton?) afterwards they need to make peace or even make it a BBEG.
