If the party starts one battle right after finishing another one, do you re-roll initiative or the players keep initiative from the first battle, and the new enemy just rolls its initiative and gets added to the order accordingly?
Whichever you prefer, but note that some classes (bards, monks, sorcerers, and some subclasses of fighters) have high-level features that trigger when they roll initiative.
I usually base my decision on whether the new battle is a continuation of the old battle (if the party encountered monster B in the process of running away from monster A, for instance) and how lazy I'm feeling.

I will look over the PHB again and see what level these initiative effects are gained. But if by "high-level" you mean in the range of 15-20, then I don't think that will be an issue.
This won't be such a lengthy campaign to warrant such high levels.
Assassin rogues are effected by new initiative at level-3. But monks and sorcerers which Mathias referenced, gain resources on initiative if they are low at level-20.
