Does it have to be initiative count 20 or can it be any number that would be in initiative order? Also, does it have to lose initiative ties? Thanks!
I think you are missing part of the question there mate. What is the "it" you are referring to?
Lair actions or regional effects.
Then the RAW answer is yes and yes. But if you are the DM then you can change what you want, just be fair and decide (and inform) before the game.
