cant find ant info on this but my question is does sap effect opportunity attacks ? for example if i hit my target with a flail and have to move away from the sapped npc to help another player would the sap effect (disadvantage on next attack roll) activate as i move away from the npc ?
As Sequilonis stated, yes it works as stated and they would have Disadvantage on the next attack (no matter what kind of attack).
This also works if they are running to attack your friend. They go to leave combat with you and trigger and Opportunity Attack, you land the attack and Sap would give them Disadvantage on the attack towards your friend, thus possibly saving a squishy caster, or someone low on HP.

Yes, it works on their first attack roll of any kind, including opportunity attacks and spell attacks.
