1) Yes, use your DEX mod. You always add it to weapon damage unless the feature for a given attack says otherwise- default Light, Cleave, etc.
2) Technically I don’t believe it works under RAW- the Soulknife feature specifically says they only appear from the Attack Action or AoO Reaction and vanish immediately after the attacks have resolved. That said, it’s pretty likely a DM will still allow you to use the feature via summoning the weapons for it.

Yeah, given that Defensive Duelist uses your Reaction, it's essentially equivalent to an Opportunity attack in terms of action economy, so it seems quite reasonable to allow.

some quick questions hopefully.
1) on the bonus attack that uses 1d4 do i still add my dex modifier to my damage roll or do i need the dual wielder feat?
2) the blades spawn for an AoO can they spawn for defensive duellist?
