You also have to remember that in 90% of any D&D campaign the vast majority of people are actually rural agriculturalists well used to living close to the land. They would have a fairly good idea of what wild foods to eat or not eat in their local area.
Granted the vast majority of city folk in D&D would have the same problem surviving in the wild as modern people do. But they would not be the people traveling the wilderness. And if they were they would look like some rich British adventure archeologist out if the 1800's going to Africa or Egypt. 5 English guys and 50 porters. Or they would be in small groups traveling from known campsites(water) to known campsites(water). Or inn to inn as many think, this is a more modern thing though.
I could see locals farmers having better hunter gather skills(survival) than adventurers from outside the area. Who would think that you could find mussels, clams and oysters under the leading edge of glaciers at low tide along the ocean coast? The locals know.
