So I’m looking for suggestions on a way to penalize my character for role playing purposes. He was killed during a fight after getting stuck in a web and being swarmed by a horde of smaller spiders. The party eventually killed everything and they took him to a temple to get resurrected and he was. He suffered nightmares about having spiders all over him but I figure that his demise should have a tangible effect for awhile at least. I was considering talking to the DM about possibly having him make a save if he fights spiders again or maybe be frightened by them? Any other suggestions? I don’t want anything too debilitating but I feel there should be a cost because of how it went down.
What you have there sounds good to me. Possibly also if he gets restrained in a net or another Web, he'll start to panic and not be able to think rationally.
Glad to help. It warms my heart to hear another player who is willing to take on something that lowers their character's optimized readiness in favor of role-playing. I tend to create extensive backgrounds for my characters, so I know where you're coming from with that question.

I played a half-elf cleric that was used to the convenience of city living. As such, he didn't skip meals, didn't sleep on the ground or in his armor, used a lantern in dark areas (despite having darkvision), and never learned to swim. Planned on having him work through those things as he adventured. Alas that campaign was abandoned.
If you look in the DMG there are tables for short-term, long-term, and indefinite Madness. The player could either roll on one of these tables or the DM could pick an appropriate form of madness which will reflect a RP (or combat) effect that is a result of the character's nightmares

Thanks for the suggestions. After talking it over with the DM it’s going to be a save using an average of WIS (act of will) and INT (reasoning ability). So it’s a matter of fighting thru something with fortitude and strength of will but also reasoning himself out of an irrational fear. In my case with my stats the bonus would be +2. Leaving it to him as to when the save would occur and the extent of the panic, if any. I like the fact that there are real consequences to the “death.” The resurrection was physical but left mental scars. It’ll be interesting to role play it.

I'd recommend just roleplaying it, not invoking random saving throws on yourself.

Adventuring Options in the DMG has a handy Lingering Injuries table.
