You select a sphere's point of origin, and the sphere extends outward from that point. The sphere's size is expressed as a radius in feet that extends from the point.
A sphere's point of origin is included in the sphere's area of effect.
So all the Xft sphere descriptions mean a sphere with a radius of X feet, not a diameter.

The text is the 'primary source', so it's a 20' radius.
It's a 20' radius sphere.
The picture is just to show you the shape. The description will tell you size.

its 40 foot from end to end, 20ft from the center point in all directions. unless it hits structures.
