After researching from 2e material to PHB 2024 I have the following theory where The Spire in the Outlands is the key and Sigil “The City of Doors” is the lock. This avoids creatures from outside Great Wheel Cosmology (an other known cosmologies previously presented in other editions) from Far Realm and other realities to enter and affect the known multiverse.
This could be one of the reasons why the Lady of Pain, ruler of Sigil is very enigmatic and avoid interractions like being worshipped or keeping social relations. Appart from protecting the City from threats against her ruiling she could be protecting something else. This is also why the spire, the closer you get, the more restricted rhe magic becomes.
Additionally, in material as “Doors to the unknown” 4th module and “A guide to the outlands” from 2e. They mentioned beings of higher realities, I believe that the first one could be related to higher dimensional (geometrical) beings that could be beyond Far Realm Comprehension. And the second where the Hinterlands are mentioned, I could relate them to a “space” of shifting gate towns of unknown planes outside great wheel cosmology. The mist in the Hinterlands could be the last attempt by an outside power to make an incursion in the multiverse.And these are how some portals in Sigil give this hint and how Lady of Pain avoids major involvement.
