Some ancient stories do describe her as a dragon, or a sea monster. The five-headed part may be TSR but they did go into some very obscure areas of mythology for the appearance of some of their descriptions in the original monster manual so the specific look they went for could have some very ancient origins. Anyway, I think that all the non-D&D Tiamat stuff that is out there is well worth going ito as a DM if you are looking for some plot inspiration. Or just looking for an explanation as to how she ended up in Avernus in the first place and who put her there.
