In an age past, (4e) Reihon Lifewhisper was a great mage reaching the heights of the epic tier. In a final attempt to buy enough time for Brother Red to complete his teleport spell to transport the innocent civilians and faction leaders he reached deep into his Blood Mage roots to hold the demons at bay, obliterating half of them in the process. Calissa (5e) was given a small crystal by a strange old man as a graduation gift from the wizard academy in Candlekeep. For years Calissa traveled Faerun trying to figure out what this crystal meant and what was inside it, as she could sense it's power and it was always freezing cold!

Calissa has recalled her ancestor Reihon as her Simulacrum to guide her through the challenges to come in this age's epic tier. She also, dabbled into fighter in a final preparation for what is to come!
level 14 wizard - evoker (overchannel feature), level 2 fighter (action surge) human - alert feat, also elemental adept - cold very high bonus to initiative tends to lead to going first.
Move: Into optimal positioning. Action: Cone of Cold (overchannelled) for 69 damage dc 18 con save to all enemies, sculpting around allies. Action Surge: Cone of Cold (overchannelled and take 2d12 damage) for 69 damage dc 18 con save to all enemies, sculpting around allies. Simulacrum Acting on your Initiative - Move: into optimal positioning. Simulacrum Action: Cone of Cold (overchannelled) from spell storing ring for 69 damage dc 18... you get the idea.
This alpha strike leads to 207 damage, save for half. It's a pretty fun opener! If you're simulacrum still has their action surge, then that's 276 damage save for half... Oh and it penetrates cold resistance.
Variant Human Blade-Pact Hexblade* warlock. 1st-level feat Spell Sniper, which lets you learn 1 cantrip of your choice that requires an attack roll (eldritch blast). Start with 16 Cha, 14 each Dex and Con. Hex Warrior* gives you proficiency with martial weapons, medium armor, and shields; and lets you use Charisma instead of Strength or Dexterity for melee weapons that aren't two-handed. Invocations in approximate order: Agonizing Blast, Repelling Blast, Thirsting Blade, Eldritch Spear, Frost Lance*, Lifedrinker. Max Charisma as soon as possible. Straight warlock up to 14th level, then whatever you want.
This leaves you with 1 ASI/feat; 14 levels of warlock spellcasting; your background, class, and race proficiencies; and your 15th-20th levels to do whatever you want with.

if you max out dex (+5)
take the Alert feat (+5)
take the swashbuckler roguish archetype, that allows you to add your Charisma mod to initiative (+5)
then grab 2 levels of bard for 1/2 of your proficiency bonus to initiative
5 (Dex) + 5 (Alert) +5 (Cha) +3 (jack of all trades) = 18
if your dm allows Unearthed Arcana you can take a level of Ranger Revised for advantage on initiative or take 7 levels of barbarian
+18 w/ advantage has rarely let me go second during combat
Variant human (take war caster), 1 level of warlock, hexblade (booming blade is a must have) 7 levels of paladin tunnel fighting and oath of vengeance
If an opponent moves 30 ft from adjacent without taking disengage, 6 4d8+modifier attacks can be landed, increasing in damage as you level.
