Just as the title says, I am not sure if it's possible to customise the tattoo so that it shows the spell and let's it appear in the character spell list or something... Anyone knows if it's possible and if so, how to do it? I'm kinda new to DnD Beyond so please walk me through the process if doable :)

The D&D Beyond character builder doesn't have any proper support for that, unfortunately. I think the best you can do is customize the name of the item in your inventory.

You will probably have to create a homebrew magic item (the tattoo) for the campaign that character is in for that to work. Necklace of Prayer Beads is probably going to be your best base for this, as that item literally adds spells to your spell list on the DDB character sheet.
So go to Collections, Create Magic Item, select Wonderous item and then Necklace of Prayer Beads, create. It'll create a copy of that, which you can modified how you want.
I've not messed with that specifically, but that is probably your best shot to get it to work. I'm guessing the spells section at the bottom (modifiers, Conditions, Spells) is where the spell gets added. Can't see anywhere else that specifically tags it to be listed in your spell list.

I'd play around with that and see if it works.
I went the necklace of prayer beads route like Deyric said, and it might be easier to
1. create a custom homebrew item using Deyric's steps to "go to Collections, Create Magic Item, select Wonderous item"
2. add in your description
3. add your spell attached to it at the bottom
4. add a charge and specifics about it
I don't have a subscription, if you do, you may be able to just utilize someone else's homebrew tattoo/item.
