I had an idea surge through my berserk mind. A 2d plane. A plane with multiple layers.
The 2d plane, in essence, is exactly how it sounds. It's a plane that is entirely two-dimensional. Left right. Up down. That is all the inhabitants of this flat world know. It is a plane like any other, and has its own rules and portals. 3d creatures have..... trouble transitioning into this strange world. Bizarre deformities afflict those who go here, though they are reverted back to normal in the 3d world. Creatures from the 2d plane can go into the 2d world and take on a 3d form, but they are so utterly bizarre here that they are classed as Aberrations. Too someone in our world, everything is pixilated. The first layer is known as the Veil. The Veil is called this because it is the most complex of this plane. Abstract versions of beings can be found here. It is, by all accounts, the most similar to our world. Flight is meaningless here, because up down left and right are the only directions. Portals in this realm can be found, which lead down into the Realm.
The Realm is the largest layer of the 2d plane. In this layer, things are simpler. Only shapes (or shapes shaped into more complex forms) exist here. It's simple, and strange.
Then is the Threshold, the 3d layer. Here everything is distorted. Life barely treads this layer. There is much speculation. Many sages dare not tread here for risk of transitioning into the final layer. It is barren and dead. Extreme beings do exist here, though they are very abstract to us.
Finally, there is the Event Horizon, the final layer. It is called this because once crossing over here, it is impossible to go back. No mortal, and even some godly magic, can aid in escape from this prison of a plane. It is actually just as complex as the Veil, but is a red in color. One being is known to live here. Old Man Maverick. Maverick is supposedly a being who actually originated on the Event Horizon, which is odd. The only creatures living here are trapped beings, doomed to wander the crimson lands. He is always found fishing in a 2d lake, sitting contently. He says he does not remember where he came from. He just remembers being here, and living out his strange, imprisoned life. He yearns to see the upper layers of the plane and maybe even the 3d world, which the trapped adventurers and wanderers tell him about. But for now, he's content to fish.

There is a book from long ago about a 2-D universe. The book is called Flatland, and I think it was written by a fellow named Abbot. There is not much in there you can use to make a D&D world, but it is a great description of the problems we would have interacting in a 2-D world.
