A underdark theme would be really cool, because there is a ton of awesome things for it many often don't get to use. Then we could have a progression of bosses, or even just Iconic foes for the final boss such as an elder brain, Lich, or similar-themed creature. Also, I don't think there's a creepier setting than that.
