As far as I can tell there is no penalty for attacking into, out of, being the grappler or the grapplee. Except that the grappler needs to keep one hand on the victim.

Nothing special in particular happens in that situation. You roll, you hit, you do damage, and the grappler continues grappling, assuming it's hp didn't go to 0 or otherwise did nothing special to it.

what is the actual rule for attacking into a grapple?

Do you mean attacking a creature with the intention of grappling it, or attacking a creature that is grappled?

The Grappled condition is a lot more basic than its name and appearance would lead one to believe.

a party member is grappled by a gargoyle and I attack the gargoyle.
