Just add the fact that you are now a werewolf, and the stats for when you are in werewolf form. Apologies if this info is incorrect, i am not the leading expert in lycanthropy.

First off, yes, you have a Curse, and those rules can be found in the 5e.24 PHB Curses section, and also some additional info in the 5e.24 DMG Curses section.
From the MM, the WerewolfStat Block will have some helpful info as you can now change at will. When you change into a Werewolf, you would use this Stat Block for your character.

In 5e, Being "cursed" with Lycanthropy is basically an upgrade as it boosts your stats, and eventually being a werewolf doesn't even matter because your human form is just going to be more powerful.
Although technically you should change your alignment to chaotic evil, but really the whole thing doesn't make sense. Really you should ask your DM about this though.

I am in the Curse of Strahd campaign and was cursed with werewolf lycanthropy.
I am a Loxodon fighter at level 3. My species seems to have mostly the same abilities. What changes on my character sheet?
