I'm sure there’s a perfectly good explanation for this, but I can’t figure it out.
I have a lvl. 2 Rock Gnome wizard, school of necromancy with a “haunted one” background. I’ve chosen skill proficiencies in Arcana, Investigation, Religion and Medicine. My point buy stats are str. 10, dex. 14, con. 14, int. 15, wiz. 10 and cha. 8.
Now, when I plot all that into the character builder, it gives me +5 in all the skills above. And I can’t figure out why? The other character builders I’ve tried (on iOS) says I should only have +4. I’m really trying to learn here!:)
Yeah medicine is the only one that goes off wisdom as opposed to the other three you listed. Calyph is right, don't forget about your racial stat boosts!

I just put those stats in to a point buy calculator, and with your racial bonuses it puts your stats actually at:
With these stats, it would explain your +5 in most of your chosen skills, except for Medicine, which should be at a +2.
