Awakened Shrub's Berry

This berry is a magic item.
As a Bonus Action, you can eat it or feed it to another creature within 5 feet of yourself. The creature that eats this berry regains 2d4 + 2 Hit Points.

Warning: eating more than 1 of the awakened shrub's berries per day may make you sick. If you eat a second one, roll a d4. 
1- blinded until next long rest (automatically fail any ability check or attack roll that requires sight, attack rolls against you have advantage)
2- poisoned until next long rest (disadvantage on attack rolls and ability checks)
3 - you take a level of exhaustion until next long rest (disadvantage on ability checks)
4 - you vomit but feel completely fine afterward
