It is actually a Portable Hole rolled up in the arrowhead, which is pushed back into a small extradimensional space in the shaft. There is a safety for the arrow, and, if the safety is on, can otherwise be used as a normal arrow. When the safety is turned off, it becomes active, and upon impact, opens up a portal to a random place on the astral plane.
IMPORTANT NOTE: To DM's: if the party member places this arrow into an extradimensional object, such as a Bag of Holding, Portable Hole, or Handy Haversack, it is counted as an Extradimensional object as well, meaning it has the same effect as if a portable hole is put into a Bag of Holding, or if the arrow's safety is off. Be careful how you use it, as it can totally derail your campaign.
Notes: DON'T GIVE THIS TO PLAYERS WITH THAT ONE MAGICAL QUIVER THAT I CAN'T REMEMBER THE NAME OF UNLESS THEY KNOW THE CONSEQUENCES
