Wondrous Item, legendary (requires attunement by a character who is either a Celestial, Fiend, or Aberration, which has access to either the Spellcasting or Pact Magic class feature)

The Seal of the Apocalypse appears on the eye of one meant to bring destruction. This eye always looks towards the destruction of certain creatures, intent on wiping them from the mortal plane.

While attuned to this item, you have a +2 bonus to Spell Attack Rolls, and the Saving Throw DC's of your spells. This does not stack with other caster attunements.

When an Apocalypse Seal is created, the DM chooses three creature types.

When at least one of the targets of a spell you cast is a creature of the given type, increase the bonus to your spell attack rolls and Saving Throw DC's provided by this item to +3.

When a creature of one of the chosen creature types dies within a minute after being damaged by one of your spells or failing a Saving Throw from one of your spells, the seal shatters their soul, absorbing part of it's essence. Every time the seal absorbs 20 souls, it permanently gains one of the following properties, determined by rolling a 1d100. If you roll a property the blade already has, roll again.

After it gains it's first property, it no longer feels satisfied with low level creatures. The Seal will only absorb the souls of CR 15 or greater creatures.

After it's second property, the Seal will only absorb the souls of CR 20 or greater creatures.

After it's third property, the Seal will only absorb the souls of CR 20 or greater creatures that have Legendary Actions.

When the Seal would gain it's fourth property, it automatically gains the final property on the table below.

1-40 Further increase the Spell Attack Bonus and Save DC bonuses granted by this item to +4, if the spell targets at least one of the chosen creature types.
41-60 While attuned to the seal, you learn the spell Counterspell, and always have it prepared. Once, when you successfully Counterspell a creature of the given type, regain a spell slot equal to or lower than the level of the spell countered. If the spell countered was a level 7 or higher spell, regain a level 7 or lower spell slot instead. Regain the use of the feature at the next dawn.
61-70 When you cast a spell that only targets creatures of the chosen type, you may forgo using a spell slot. Instead, spend and roll a number of unspent hit dice equal to twice the spell's level, and take that amount as necrotic damage. This damage does not force a concentration check, and cannot be reduced or negated in any way.
71-80 A cult has begun to pray for your success after seeing the power of the seal, giving you a fraction of the power of the gods. Once, when you would roll a d20 Test, you may choose before the roll to treat the result as a 20. Once you do this, you must wait 7 days before you can use this feature again. 
81-85 The Seal has gifted you with Eldritch Knowledge from it's creator. Choose a level 4 or lower spell from any spell list. While you are attuned to this item, you have that spell prepared. It counts as a class spell for whichever class you have the most levels in.
86-90 While attuned to the Apocalypse Seal, gain a +2 to all Saving Throws.  
91-95 When you cast a spell that targets a single creature, you may have it instead it targets all creatures of the chosen types within the spell's range. For each creature targeted, you take damage equal to your Spellcasting Ability Score. This damage bypasses immunities and resistances, and does not trigger a Concentration Saving Throw.
96-99 As a magic action, you may bring the Apocalypse to all creatures of the chosen types within 5 ft. of you. Each creature of the given types within 5 ft. of you immediately takes radiant damage equal to half of it's maximum hit points, and you immediately die, your body disintegrating completely. This action destroys the Apocalypse Seal, as well as all equipment on your person. When you use this action, your death cannot be stopped by effects that cancel instantaneous death effects. 
100 The Seal determines itself to be complete - and unattunes itself from you. It sends the absorbed souls back to it's creator. If you are a celestial, you immediately become a Transformation Level 4 Seraph. If you were a Fiend, you become a Transformation Level 4 Fiend. If you are a Aberration, you become a Transformation Level 4 Aberrant Horror.

 

Cursed: While attuned to the Apocalypse Seal, you cannot willingly unattune to it. Breaking this attunement requires the use of the Wish spell, which you cannot do yourself. While attuned to this weapon, it twists your alignment - you seek and look for any reason that would fit within your given alignment and morality to kill creatures of the chosen types. Additionally, if you would cast a spell on a creature of the given types that would not do damage or require a Saving Throw, you must use a spell slot double the level at which you are attempting to cast that spell (you do not gain any upcasting benefits from this). If you are unable to meet this requirement, the spell is countered as if you had a failed a save against the spell Counterspell.
