This is an inhaler, just reshaped into a fantasy setting. This item comes with 3 charges and needs to be refilled manually by either an Artificer who is proficient with Alchemy, or a Cleric. When you are afflicted with the Breathless CONDITION, you take a puff, and the CONDITION is gone. But if you MISS it, the CONDITION worsens over time/with each missed CON Save/combat turn taken WITHOUT taking a hit of your inhaler.
To take a hit of your inhaler is a BONUS ACTION.
