The Infernal Die belongs to the Dice Demon. When it is bored, it bestows it on unsuspecting adventurers, giving them the ability to nudge their fates.. at a great cost.
The Infernal Die is a D6 with all blank sides. As long as the Infernal Die is in the possession of the party, any player may choose to roll the Infernal Die instead of any normal D6.
Whenever a blank side is rolled, the player chooses the result. It can be any number. The result is then written on the side that was rolled. Whenever the Infernal Die hits a side with a result written on it, that result counts.
As soon as the last blank side is rolled, the party is teleported into the lair of the Dice Demon, the final blank side is permanently engraved with the Mark of Misfortune. And the Infernal Die returns to the Dice Demon, who then uses it against the party. If the Dice Demon is defeated, the Infernal Die returns to the party. All numbered sides are erased and only the Mark of Misfortune remains.
When a Mark of Misfortune is rolled, the player that rolled it rolls a 1d6 *d6 damage. However, the Dice Demon (or rather the DM) can force a reroll on one die roll of his choosing for every time Mark of Misfortune is rolled.
