This innocuous looking jar of Three blind eyes loading in a green liquor. Upon saying the command word blind. The jar will cast the spell Blindness/Deafness. Up to three times per long rest. Once per each eye with a Chance of blinding yourself at a 30% chance. For each use it will always cast the blind, not deafness
