Amulet of the Beast

This amulet hangs from a delicate adamant chain. Made from scrimshaw, the shape of different beasts can be found in its intricate carvings. 

Once attuned, the bearer can use the  beast form feat three times per day. When using the amulet in this manner, the first use can be for any beast of CR 2 or less. The subsequent uses of that day are limited to that beast.

After each use, the  bearer must succeed in a constitution save with a DC equal to the total number of times used by that character. If they fail,  the character contracts lycanthropy and using whichever beast they chose during that use.  This disease proceeds according to the usual rules. After contracting the disease, the Amulet ceases to function, but requires a DC 15 wisdom save @ disadvantage to be removed.
