Wondrous Item, rare (requires attunement)

This green crystalline shard is an ancient relic imbued with necrotic energies long ago by Asmodeus himself.

Its master and wielder, once attuned, can attempt to magically implant the shard into the chest of any living creature. The master must make a spell attack against the targets armour class and attempt to press the shard into the targets chest.  for every spell level slot above a 1st level slot, add +2 to attack score per spell level slot.  Undead and constructs are immune to this item. If the target is incapacitated the attempt automatically succeeds.

Once the shard is embedded, as an action the shards master can use it to transfer 3d6 Hit Points from the victim to them-self if it is within 60 ft. the shard can only be embedded once per day, the ability recharging at midnight.  If the targets hit points are reduced to 0 from a use of the shard, then the target instantly dies and their body shrivels to a mummified husk

The shard exerts no control over the victim otherwise, but the master of the shard knows it’s direction and distance at all times. If the shard crosses into another plane, the owner knows which one.

Extraction. A skilled physician can remove the gem on a successful DC 17 Intelligence (Medicine) check, but the patient suffers 4d10 piercing damage on every attempt. A greater restoration spell or wish spell can be used to eject the gem.  The master can also attempt to remove the shard from the living target utilising the same procedure for insertion, however if the target wishes not to contest the action, the attempt automatically succeeds.

The emerald radiates evil, and should its ‘master’ ever use it for it’s intended purpose, their alignment permanently shifts a step closer to evil.
