The origin of this map has been lost to history, however, what has been discovered is that it is actually one of two such maps. The Adquisitrix Aequar, which translates from core primordial to the merchant's tongue as Acquirer of Seas, looks like a weathered map of an ocean until you realize the image shifts. Not only in that the drawings on the map seems slightly animated (waves moving for example) but that it alternates between the maps of various bodies of water.

Submerging the map in water attunes it to the source of the water used. For example, placing the map under water from a specific lake, stream, ocean, will make the map will focus its imagery on that region. One does not even need to be in that specific region as long as you have a container with uncontaminated water from the location. Once attuned, it shows you the geography of the region and it's immediate surroundings. This includes artificial structures like temples, cities, walls, ect. It does not label any of it though so one does not always know exactly what they are looking at. Using the map within the attune region provides expertise on Survival checks used within the area, including for navigation and not getting lost.

Ancient texts mention that more details can be revealed by the map.

Intelligence (Religion) DC 20: Passing this check reveals the following details. You can have a Sage or other NPC attempt the check but each creature can only attempt it once. As a divine artifact, this feature remains hidden when using the Identify spell. If you are actually at the location and not just using previously recovered water from the source, mixing one's own blood into the water as you attune it absorbs some of your own life force. Your Constitution temporarily lowers, but the map zooms in to a few miles area centered on the you. You must keep the map on your person and maintain concentration, but the map now reveals living creatures locations in real time. As you invest the blood you choose how much to lower your Constitution by, and the details provided by the map are explained in the table below.

The penalty, and added details last until your next long rest and cannot be undone prematurely by magic other than the Greater Restoration or Wish spells.
