Alteration Scroll

Use to create Alter someone. Roll a DC 20 Con save (half DC if you have either the Alteration Wand or the Alteration Potion, and if you have both, no save needed!)

if failed, the target takes 5d12 damage and falls unconscious.

if succeeded, (or you just got all three items,) you Alter the target. Roll on the Alteration table. (in the Altered background [#ShamelessSelfPromotion] you do not gain the actual background, just the Alteration.)
