Wondrous Item, very rare

A large red ruby sits in the mouth of a black skull with two elongated fangs at the end of a black chain. There seems to be mist or light swirling within the ruby and when in combat or use the ruby pulses with a red light. The skull is affixed by a small black chain that appears delicate but cannot be scratched, broken, or defaced.

The wearer of this amulet gets resistance to necromantic magic and can cast vampiric touch (4d6 necrotic damage) 3x daily. In addition, the wearer can regain half of the damage dealt as health. The amulet also gives +5 HP and grants the bearer the ability to read, write, and understand infernal.

Curse: The ruby is actually a prison for a demon. Once the wearer puts it on, they hear slight whispering which occurs outside the range of discernable words whenever they are anywhere silent. The more the wearer casts vampiric drain, the more bonded they become with the demo. Eventually the words become distinguishable by the demon telling them to feed and to consume blood. The amulet also confers +8 HP at this point. After the sixth cast of vampiric drain, the demon converses with the wearer directly and tries to convince the wearer to find a way to release it. (Charisma 20)

The wearer cannot remove the amulet unless remove curse is cast. After every long rest the wearer must make a DC 15 CON save or else take 4d6 necrotic damage with half going back to the wearer (resistance is applied) if they have not used it in battle recently.

If the wearer falls below 10 hp they MUST cast Vampire Touch on the nearest PC or NPC.

After the sixth cast of Vampiric Drain, the DM rolls to determine WHO the spell affects (whether its enemies, NPC, or PC’s) and the bonding with the demon becomes strongest and the demon begins to try and convince the wearer to free it.
